What the Arizona rental application is
This is a two-page Arizona rental application in the style used by many apartment communities. Page 1 is the application itself: applicant, occupants, pets, two years of residence and employment history, spouse or co-occupant details, banks and credit references. Page 2 adds driver's licenses, vehicles, background questions and emergency contacts, then a Deposit to Hold Agreement and a Rental Agreement Information table that lists the rent, deposits, fees and taxes due at move-in. It is a general template, not a form issued by the State of Arizona.
Who uses it
Arizona landlords use it to screen applicants and to record a holding deposit and move-in costs in one place. A separate application is required for every adult except a spouse.
What to have ready
- Social Security numbers for you and any listed occupants, plus driver's license numbers, states and expiration dates for you and your spouse
- Addresses, length of stay and landlord name and phone for at least the last two years
- Employer, address, phone, position, supervisor and monthly income, plus former employers if you have been in your job less than two years
- Bank names, account numbers and branch addresses, and credit card or charge account details with expiration dates
- Make, model, year, color and plate for every vehicle you want to park on the property

How to fill out the Arizona rental application online

Sections 1 to 3: applicant, occupants and pets
Section 1 asks for your Name, Married or Single, Date of Birth, Present Phone No. and Soc. Sec. No. Section 2 lists other occupants (a, b and c) with Name, Relationship, Age (if under 18) and Social Security No. Section 3 asks whether a pet or assistive animal of any type will live in the apartment and, if yes, its Type, Weight (Full Grown), Spayed/Neutered, Licensed/Date and Breed, listing all significant blood lines for a mixed breed.
Sections 4 to 6: residence and employment
Section 4 covers your Current Residence and up to two Former Residence entries if you have lived at your present address less than two years: Address, Apt No., City/State, Zip Code, How Long (years and months), Name of Landlord and Landlord Phone. Section 5 asks for Employed by, Address, Phone, Position, How Long, Supervisor's Name, Your Monthly Income and Other Source(s) of Income for Rental Payment, with two Former Employer blocks for anyone in their job less than two years. Section 6 repeats the identity and employment fields for a spouse or other occupant.
Sections 7 to 13: banks, credit, vehicles and background
Sections 7 and 8 list your banks and credit references (Type, Bank/Store/Company, Card/Account No. and Expiration Date). Section 9 covers driver's licenses and every vehicle you want to park, including boats, trailers or RVs, which need separate written permission. Section 10 asks about evictions, bankruptcy, illegal drug use or distribution and certain criminal convictions, with a line to explain a yes. Sections 11 to 13 ask about outstanding warrants and waterbeds and name the person to notify, and to take possession of your property, in an emergency.
Deposit to Hold Agreement and Rental Agreement Information
Management fills in the holding deposit, the non-refundable administrative processing fee and the cancellation deadline (5 p.m. on a stated date). Under the form's terms, the holding deposit is refunded if you are not approved, credited to move-in costs if you are, and forfeited if you cancel after the deadline or do not pay rent by the rent start date. The Rental Agreement Information table then lists the Apt. #, Type, furnished status, Agreement Length, Rent Start/Ending Date, who pays utilities, monthly charges and every item in the TOTAL DUE AT MOVE-IN.
Signing and submitting
Your signature confirms the information is true, authorizes verification of references and credit, and accepts the Deposit to Hold Agreement. Sign, date and return the PDF to the leasing office; the Management's Receipt line is for the landlord. You can also sign a PDF online if the property sends its own version.
Arizona rules on fees, deposits and screening

- Application and non-refundable fees: Arizona does not cap application fees. Under A.R.S. § 33-1321(B), the purpose of every non-refundable fee or deposit must be stated in writing by the landlord, and any fee or deposit not designated as non-refundable is refundable. The form labels its processing fee and charges accordingly.
- Security deposit limit: Under A.R.S. § 33-1321(A), a landlord may not demand or receive security, including prepaid rent, of more than one and one-half months' rent.
- Deposit return: Within 14 days, not counting weekends and legal holidays, after the tenancy ends, possession is delivered and the tenant asks for it, the landlord must provide an itemized list of deductions and any amount due. A landlord who does not comply can owe twice the amount wrongfully withheld.
- City rental tax lines: The form has lines for Sales Tax and City Sales Tax on rent. The Arizona Department of Revenue announced that cities and towns can no longer levy transaction privilege tax on residential rentals as of January 1, 2025. If an amount appears on the City Sales Tax line, ask the landlord what it covers.
- Screening reports: If a landlord denies an application based on a credit or background report, the federal Fair Credit Reporting Act requires an adverse action notice. See the FTC's guidance for landlords.
Common mistakes
- Skipping the Former Residence or Former Employer blocks when you have been at your address or job less than two years
- Leaving the cancellation date in the Deposit to Hold Agreement blank
- Not checking TOTAL DUE AT MOVE-IN against the quoted charges

Is this an official Arizona rental application?
No. Arizona does not issue a state rental application. This is a general Arizona-style application template that you can fill in, sign and download as a PDF.
