What Form 1040-SR is

Form 1040-SR, U.S. Income Tax Return for Seniors, is an optional version of the federal individual income tax return for older taxpayers. This template is the IRS 2025 edition, a four-page fillable PDF for income earned in 2025. As of October 3, 2026, it is the current final edition on the IRS Form 1040-SR page. The IRS has posted a 2026 draft, which is not for filing.

The IRS says Form 1040-SR uses the same schedules and instructions as Form 1040, and the line numbers match. The differences are in layout. The lines are spread over four pages instead of two, and page 4 holds a Standard Deduction Chart, so you can read your deduction off the form rather than looking it up.

Who can use Form 1040-SR

For 2025, the Form 1040 and 1040-SR instructions say Form 1040-SR is available to you if you were born before January 2, 1961. It is optional: everyone can file Form 1040 instead, and the tax result is the same.

What to have ready

  • Social security numbers for you, your spouse, and any dependents
  • Form SSA-1099 for social security benefits (lines 6a and 6b)
  • Forms 1099-R for pensions, annuities, and IRA distributions (lines 4a through 5b)
  • Forms W-2 if you still had wages, and Forms 1099 for interest and dividends
  • Records of 2025 estimated tax payments
  • Bank routing and account numbers for a direct deposit refund

How to fill out Form 1040-SR online

Click "Fill Template Now," click into each box and type, tick the checkboxes that apply, and download the completed PDF. The template does not do the arithmetic, so work out each line using the instructions.

Page 1: personal details and wages

Enter your name, SSN, and address (and your spouse's on a joint return), the Presidential Election Campaign boxes, one Filing Status box, the Digital Assets Yes or No question, and up to four dependents. Lines 1a through 1z cover wages and other earned income.

Page 2: income, deductions, and tax

  • Lines 2a through 7a: interest, dividends, IRA distributions (with Rollover and QCD boxes on line 4c), pensions and annuities (with Rollover and PSO boxes on line 5c), social security benefits, and capital gains.
  • Lines 8 through 11a: Schedule 1 income and adjustments, total income, and adjusted gross income.
  • Line 12d: check "Were born before January 2, 1961" and "Are blind" for yourself and your spouse as they apply. The number of boxes you check drives the chart on page 4.
  • Lines 12e through 15: standard or itemized deduction, the qualified business income deduction (13a), Schedule 1-A deductions (13b), and taxable income.
  • Lines 16 through 24: tax, credits, other taxes, and total tax.

Page 3: payments, refund, and signatures

Lines 25 through 33 cover withholding, estimated payments, and refundable credits. Lines 34 through 36 handle your refund and direct deposit, lines 37 and 38 any amount you owe, followed by Third Party Designee, Sign Here, and Paid Preparer Use Only.

Page 4: Standard Deduction Chart

The chart on the 2025 form lists these amounts by filing status and the number of line 12d boxes checked:

The form says not to use the chart if someone can claim you (or your spouse on a joint return) as a dependent, your spouse itemizes on a separate return, or you were a dual-status alien. The separate enhanced deduction for seniors, up to $6,000 per qualifying person for 2025 through 2028 according to the IRS, is figured on Schedule 1-A and entered on line 13b, not in this chart.

Signing and mailing

The IRS instructions say a Form 1040-SR filed on paper must be signed by hand; digital, electronic, or typed-font signatures are not valid on paper returns. Print the completed form, sign and date it in ink (both spouses on a joint return), attach Forms W-2, and Forms W-2G and 1099-R if tax was withheld, and mail it to the same address used for Form 1040, listed by state on the IRS page Where to file paper tax returns.

The 2025 return was due April 15, 2026, or October 15, 2026, for most calendar-year filers who requested an extension with Form 4868, per the IRS and the 2025 Form 4868 (as of October 3, 2026).

Common mistakes

  • Forgetting to check the line 12d boxes, which lowers the deduction shown in the chart.
  • Using the chart when someone can claim you as a dependent.
  • Putting the $6,000 senior deduction in line 12e instead of Schedule 1-A and line 13b.
  • Entering the gross social security benefit on line 6b instead of the taxable amount figured with the instructions.
