What the Nevada rental application is
This is a four-page fillable PDF titled "Nevada Rental Application," marked "Rev. 11/09." It covers move-in costs, an applicant and a co-applicant, residence and employment history, credit references, vehicles, other occupants, pets, screening questions, and a nine-point disclosure that both applicants sign. It is a general template, not a form issued by the State of Nevada. Because the form dates from 2009, some printed terms, such as the non-refundable fee language, should be read alongside current Nevada law described below.
Who uses it
Nevada leasing agents, property managers, and landlords use it to process applications, and renters use it to apply with a roommate or partner. The form says that if more than two people are applying, each extra person needs an additional application.
What to have ready
- The property address, move-in date, rent, and the fee and deposit amounts the landlord quoted
- SSN, driver's license number and state, and birth date for each applicant
- Current and prior addresses, with landlord or mortgage holder name and phone
- Current employer, and a prior employer if you have been at your job less than 3 years
- Bank account details, vehicle details, and pet information
How to fill it out online

Page 1: costs and applicant
- Required to submit: Application (Non-Refundable) Fee, Deposit to Hold, and Amount Received.
- Property: Property Address, City, State, Zip, and Move-In Date.
- Move-in costs: application fee, Rent, Security Deposit, Pet Deposit, Processing Fee, Key Deposit, Cleaning Fee, Other, and how payment was made (cash, check, cashier's check, or money order).
- Applicant: name, phones, Email, SSN, DL and State, Birth Date.
- Current Address and Prior Street Address: landlord name or mortgage holder, payment, phone, how long, owned or rent, and reason for leaving.
- Current Employer: how long, Employed As, address, phone, fax, salary per month, supervisor, and other income.
Page 2: co-applicant
The same identity, address, employment, and prior employer fields for the Co-Applicant, plus Credit References (bank, account number, address).
Page 3: household
- Automobiles: Make, Model, Lic #, State, Yr, and Color for up to three vehicles.
- Other persons to be at premises: Name, Relationship, Age, and Occupation.
- Pets: how many cats, dogs, or other, then Name, Cat or Dog, Breed, Weight, and Spayed or Neutered for up to three pets.
- Screening questions: bankruptcy, eviction, willful refusal to pay rent, and gross misdemeanor or felony convictions, each with space to explain.
Page 4: disclosure
How long you plan to stay, liquid-filled furniture, smoking, an emergency contact for each applicant, and the disclosure blanks: the leasing agent's name, property address, monthly rent, application fee, holding deposit, and the number of business days for refunds and for paying rent and deposit after approval.
Signing and submitting
Initial each page, then sign and add the date and time in the Signature of Applicant and Signature of Co-Applicant fields, which you can do with our PDF signature tool. Leave the Office Use Only line blank and send the PDF to the leasing agent.
Nevada rules on fees and deposits
General information as of October 2026, not legal advice.
- Application fees: Assembly Bill 121 (2025), effective October 1, 2025, requires a landlord to refund an application, credit report, or background check fee if it rents the unit to someone else and never did the activity the fee paid for. It also bars collecting those fees for a minor in the applicant's household. That can override this form's "non-refundable" wording.
- Deposit limit and return: under NRS 118A.242, total security, including last month's rent, may not exceed 3 months' periodic rent. The landlord must give an itemized written accounting and return any remaining amount within 30 days after the tenancy ends.
Common mistakes
- Forgetting the initials on every page, which the form says stops processing.
- Leaving the business-day blanks in the disclosure empty. Fill them in with the agent before signing.
- Skipping the prior employer section when you have less than 3 years at your current job.
- Answering the conviction question for yourself only. It covers any applicant or occupant.

Is this an official Nevada rental application?
No. Nevada does not issue a standard rental application. This is a general four-page template marked Rev. 11/09.
Can a Nevada landlord charge an application fee for my child?
No. Since October 1, 2025, AB 121 bars landlords from collecting an application, credit report, or background check fee for a minor in the applicant's household.
Do I get my Nevada application fee back if someone else gets the unit?
Under AB 121, the landlord must refund the fee if it rents the unit to a different applicant and did not perform the activity the fee was collected for, such as a credit or background check.
What is the maximum security deposit in Nevada?
NRS 118A.242 limits total security, including last month's rent, to 3 months' periodic rent.
How long does a Nevada landlord have to return a security deposit?
