What Form 1040-X is
Form 1040-X, Amended U.S. Individual Income Tax Return, is how you correct a Form 1040, 1040-SR, or 1040-NR you have already filed. This template is the Rev. December 2025 edition, a two-page fillable PDF. As of October 3, 2026, it is the current final revision on the IRS Form 1040-X page. The IRS has posted a draft Rev. December 2026 version, which is not for filing.
Unlike the annual return, Form 1040-X is not tied to one tax year. You write the year you are amending at the top. The Form 1040-X instructions say to file a separate Form 1040-X for each year you amend, and the IRS says you may file up to three amended returns for the same tax year.
Who uses Form 1040-X
According to the IRS page File an amended return, you file an amended return if you discover a change in your filing status, income, deductions, credits, dependents, or tax liability after filing. A recent example: in May 2026 the IRS noted that taxpayers who already filed a 2025 return may amend it to claim or change the "no tax on tips" deduction on Schedule 1-A.
The instructions also say what not to use it for. If you are requesting only a refund of penalties and interest or an addition to tax you already paid, use Form 843. To claim your share of a joint overpayment taken for your spouse's past-due debt, use Form 8379, Injured Spouse Allocation.
What to have ready
- A copy of the original return as filed, or as adjusted by the IRS
- A completed, updated Form 1040, 1040-SR, or 1040-NR for that year showing your changes. The instructions now require you to attach it to a paper-filed Form 1040-X.
- Any new or changed schedules and forms, plus supporting documents such as a corrected Form W-2 or 1099
- Any IRS notice about the return, if you are responding to one
How to fill out Form 1040-X online
Click "Fill Template Now," type into each box, and download the completed PDF. The template does not compute columns, so figure each amount with the instructions.
Page 1: header and filing status
- Return year: the calendar year, or the month and year a fiscal year ended.
- Name, SSN, and address: yours and, on a joint return, your spouse's, plus the foreign address fields if needed.
- Presidential Election Campaign: check only if you did not choose the $3 contribution before but now want to.
- Amended return filing status: you must check one box even if your status is not changing. The form warns that, in general, you cannot change from married filing jointly to married filing separately after the return due date.
Page 1: lines 1 through 23, columns A, B, and C
Column A is the original amount (or as previously adjusted), column B is the net increase or decrease, and column C is the correct amount. Lines 1 through 5 cover adjusted gross income, deductions, the qualified business income deduction (4a), Schedule 1-A deductions (4b), and taxable income. Lines 6 through 11 cover tax liability, with line 9 reserved. Lines 12 through 17 cover payments, including withholding, estimated payments, the earned income credit, and other refundable credits. Lines 18 through 23 work out the refund or the amount you owe.
Page 2: Part I, Dependents
Complete lines 25 and 27 (lines 24, 26, 28, and 29 are reserved) only if the number of dependents changes, then list all dependents claimed on the amended return on line 30, using the same columns as Form 1040.

Every Form 1040-X must include this part. State briefly what you changed and why, and point to any attached forms.
Signing, filing, and tracking
The IRS says you can file Form 1040-X electronically with tax software to amend a Form 1040, 1040-SR, or 1040-NR for the current or two prior tax periods. Paper filing is required if you originally filed that year's return on paper, and for tax years 2021 and earlier. On paper, the instructions say you must handwrite your signature, as digital, electronic, or typed-font signatures are not valid on a paper-filed Form 1040-X. Both spouses sign a joint amended return. Paper-filed Forms 1040-X are mailed to their own addresses, listed in the instructions and on the IRS page Where to file Form 1040-X. If you are responding to an IRS notice, use the address in the notice.
Deadlines: to claim a refund, you generally must file within 3 years after the date you filed the original return or 2 years after the date you paid the tax, whichever is later, with exceptions listed on the IRS amended return page. On processing, Where's My Amended Return says to allow 8 to 12 weeks, up to 16 weeks in some cases, and that status is available about 3 weeks after you file (as of October 3, 2026). Common mistakes
- Leaving Part II blank.
- Amending several years on one form instead of one form per year.
- Filing Form 1040-X only to fix a refund offset for a spouse's debt, which belongs on Form 8379.
- Forgetting to attach the updated Form 1040 and the changed schedules to a paper filing.
- Filing again while the first amended return is still processing, before checking its status.
