What this Louisiana rental application is

The "Louisiana Rental Application" is a one-page tenant application template marked "Equal Housing Opportunity." In a single page it collects the applicant's contact and ID details, current and previous addresses and landlords, a vehicle, employment and income, three yes/no screening questions, banking, three personal references, household size, and co-applicants. It ends with a certification, a signature line, and a deposit receipt section stating that $30 of the deposit is non-refundable and used for application and tenant screening services. It is a general template, not a form issued by the State of Louisiana or any Louisiana agency.

Who uses it

Louisiana landlords and property managers use it as a short, printable application for apartments and houses. Applicants fill it out when a landlord sends the PDF or asks for a completed application before approving a lease.

What to have ready

  • Driver's license number, issuing state, and expiration date
  • Your current and previous addresses, how long you lived at each, why you are leaving, and each landlord's name and phone
  • Your employer, position, income, employer address and phone, and time at the job
  • Bank names, branches, and account types
  • Names, years known, relationships, and phone numbers for three personal references

How to fill it out online

Click "Fill Template Now," type into the fields and tick the checkboxes, sign, and download the PDF. The form runs top to bottom like this:

  1. Applicant: Applicants Full Name, Phone #, DOB, Social Security #, Drivers License #, State, and Exp.
  2. Current residence: Current Address, City, State, Zip, Current Landlords Name and phone, How long at this address, and Reason for leaving.
  3. Previous residence: the same set of fields for your previous address and landlord.
  4. Auto: Yr, Make, Model, and State/License Plate #.
  5. Employment: Employer, Position, Income, Employers Address, City, State, Phone #, How long at job, and Other income/source.
  6. Yes/No questions: checkboxes for "Do you allow smoking in the home?", "Have you ever been evicted?", and "Have you ever been convicted?" The form notes you may explain any "yes" answer on the reverse.
  7. Move-in and pets: Preferred move-in date and Number and type of Pets.
  8. Banking: two rows for Name of bank, Branch, and Type of Account.
  9. Personal References: three rows for Name, Yrs. Known, Relationship, and Phone #.
  10. Household: Total number of adults, total number of children under 18 living with you, and Names and relations of all other applicants.
  11. Certification and signature: Applicant Signature and Date, certifying your answers and authorizing tenant screening.
  12. Deposit receipt: the landlord or representative enters the deposit amount received, and you initial the line confirming $30 is non-refundable and the balance is applied to the deposit or refunded if you are not approved.

Louisiana rules on fees and deposits

Application fees: written notice first

Louisiana does not set a dollar limit on application fees, but La. R.S. 9:3258.1 says a lessor may not require an application fee unless, before accepting it, the lessor gives written notice of:

  • the amount of the fee;
  • whether the lessor considers credit scores, employment history, criminal history, or eviction records; and
  • the applicant's option to share a good-faith statement of 200 words or less about financial hardship from a declared disaster or emergency, with a reference to the COVID-19 pandemic and hurricanes.

The notice may be electronic. The rule covers rentals of a primary residence, except owner-occupied buildings of four or fewer units. This template's $30 line states the amount but not the other items, so the remaining disclosures have to be given separately or added to the form.

Security deposit return

La. R.S. 9:3251 does not set a maximum deposit amount. It requires the deposit to be returned within one month after the lease ends, minus amounts reasonably needed to cover a tenant default or unreasonable wear. If anything is kept, the landlord must send an itemized statement. Under Act 63 of 2026, effective August 1, 2026, that statement may be sent within one month after the tenancy ends or within 15 days after that one-month date. The tenant must give a forwarding address.

Under La. R.S. 9:3252, a willful failure to comply lets the tenant recover the amount wrongfully kept plus $300 or twice that amount, whichever is greater. Failing to pay within 30 days after a written demand counts as willful.

Signing and submitting

Sign and date the Applicant Signature line, initial the deposit section, and give the PDF to the landlord. Our online PDF signer handles both the signature and initials. Nothing is filed with a state office.

Common mistakes

  • Leaving the driver's license State and Exp. fields blank.
  • Checking "yes" on the eviction or conviction question without adding an explanation.
  • Counting yourself in "Total number of adults" inconsistently with the co-applicant list.
  • Initialing the deposit line before the deposit amount is filled in.
