What the Indiana rental application is
This Indiana rental application is a two-page apartment application that a landlord uses to check an applicant's identity, rental and job history, finances, and references before offering a lease. Page 1 is a set of tables for residence, employment, credit, and vehicles; page 2 covers references and emergency contacts, a long list of general questions, and the signed agreement and authorization. It is a general template, not a form issued by the State of Indiana, and Indiana does not require any particular rental application.
Who uses it
Apartment owners and property managers use it to screen applicants, and renters complete it to apply for a specific unit. One applicant signs it, and other proposed occupants are listed on page 1.
What to have ready
- Your name, birth date, Social Security number, driver's license number, phone numbers, and any other names you have used
- Names, birth dates, and relationships for all other proposed occupants
- Your current, previous, and prior addresses with dates, last rent paid, and the owner or manager's name and phone
- Employer names, addresses, phones, supervisors, dates, and monthly gross pay for three jobs
- Bank and lender names with balances for savings, checking, credit cards, and auto loans
- Vehicle details for every proposed occupant, and contact details for a doctor, a lawyer, and your nearest relative living elsewhere

How to fill it out online
Click "Fill Template Now", type into the table cells and question boxes, sign at the bottom of page 2, and download the PDF to send to the landlord. The sections below match the form's headings.
Applicant and occupants
Enter your First Name, Middle, Last, Birth Date, Social Security #, Driver's License #, Any Other Names You've Used In The Past, and home and cell phones. Below that, list up to four other proposed occupants with birth date and relationship to you.
Rental/residence history
For your current, previous, and prior residence, fill in street address, city, state and ZIP, dates of residency (from and to), last rent amount paid, owner or manager and phone, and reason for leaving. Three short questions follow for each home: was rent paid in full, did you give notice, and were you asked to move. There is also a line for the name or names your utilities are billed under now.
Employment history, credit history, vehicles
For current, previous, and prior employment, give the employer, address, phone, your occupation, supervisor, monthly gross pay, and dates. Under Credit History, list the institution and balance on deposit or balance owed for savings, checking, credit card, and auto loan accounts. The vehicles table asks for make, model, color, year, and license plate, and the heading notes it includes vehicles of other proposed occupants.
Page 2: references and general information
References & Emergency Contacts asks for a doctor, a lawyer, and your nearest relative living elsewhere. Read the note under this table: by signing, you allow the landlord to contact these people if they cannot locate you, and to let the listed relative remove your belongings if you abandon the apartment. General Information covers late rent and eviction notices, smoking, how long you expect to rent, bankruptcy, move-in date, felony convictions, pets (type, breed, weight, and age), problems with your current landlord, why you are moving, optional other income, someone who could lend you rent money, lawsuits, anything negative a credit or criminal check might show, how you heard about the unit, your email, and an optional referral of someone else looking for an apartment.
Agreement and authorization
The closing statement authorizes a credit and criminal check and contact with everyone listed, and confirms the processing fee is non-refundable even if you do not get the apartment. Sign and date. You can add a signature with our sign PDF tool.
Indiana rules worth knowing
General information from the Indiana Code (checked October 2026). For your situation, read the statute or consult a qualified professional.
- Application fees and local rules. Indiana's landlord-tenant statutes do not set a dollar cap on application fees. Under IC 32-31-1-20, cities, towns, and counties generally may not regulate private landlord-tenant matters such as screening, applications, and landlord fees unless the General Assembly authorizes it.
- Security deposits. The security deposit chapter, IC 32-31-3, does not set a maximum deposit amount.
- Return deadline. Under IC 32-31-3-12 and 32-31-3-14, the landlord must return the deposit, along with an itemized list of any damages claimed, within 45 days after the tenancy ends and the tenant gives up possession. Give the landlord your forwarding address in writing so the deposit and list can reach you. A landlord who fails to provide the list can lose the right to keep any of the deposit.
Because the form authorizes a credit check, federal rules also apply: if a landlord turns you down based on a consumer report, the Federal Trade Commission explains that they must give you an adverse action notice.
Common mistakes
- Leaving the "Is/Was rent paid in full?", "Did you give notice?", and "Were you asked to move?" boxes blank
- Forgetting vehicles owned by other occupants
- Listing a relative without reading the abandonment clause that applies to them
- Writing net pay where the form asks for monthly gross pay
- Signing without a date
