- 1. Legal Name: First, Middle, Last, DOB, Dr Lic # and State, E-Mail Address, Other Names Used, Soc Sec #, and Applicant Phone, with a second set for your Spouse.
- 2. Current Address: city, state, zip, Landlord Phone, Landlord Address, How Long, Current Rent, whether proper notice has been given, Current Landlord/Mortgage Lender, and Reason for Moving.
- 3. Previous Address: Rental Address/Zip Code, Dates Occupied, landlord, phone, and address.
- 4. 1st Person Income and 5. Spouse Income: Current Employer, Employer Address/Zip, phone, Job Title, Date Employed, Name of Supervisor, and Gross Income, plus Other Income source and monthly amount.

- 6. Legal names of all occupants: name, relationship, and age for everyone, including minors.

- 7. Personal History: felony charges or convictions and the year, registered sex offender status, pets (kind, weight, dog breed), and whether you have been asked to move out or evicted, with space to explain any yes answer.

Nebraska rules on fees and deposits
General information as of October 2026, not legal advice. Nebraska's rules are in the Uniform Residential Landlord and Tenant Act.
- Application fees: we did not find a Nebraska statute that caps rental application fees. This form charges an initial fee plus a per-adult fee, so confirm both amounts before you apply.
- Deposit limit: under Neb. Rev. Stat. 76-1416, a landlord may not demand or receive security of more than one month's periodic rent, plus a pet deposit of up to one-fourth of one month's rent where appropriate.
- Deposit return: the balance and a written itemization of any deductions must be delivered or mailed within 14 days after the tenancy ends.

Common mistakes
- Leaving the "Address I am applying for" line blank, which can delay processing.
- Listing only adults in section 6. The form asks for every occupant, including minors.
- Answering yes in Personal History without filling in the explanation lines.
- Forgetting the dog breed verification the form requires.
