What Form 4547 is
Form 4547, Trump Account Election(s), is the IRS form used to open an initial Trump account for a child and, for children who qualify, to elect the one-time $1,000 Pilot Program Contribution. The IRS About Form 4547 page describes it as the form taxpayers use for elections to open an initial Trump account and to request the $1,000 pilot program contribution. This template is the December 2025 edition, which is still the current revision as of October 3, 2026.
Trump accounts were created by the law the IRS refers to as the Working Families Tax Cuts. According to the Instructions for Form 4547, no contributions can be made to a Trump account before July 4, 2026.
Who files Form 4547
The form is completed by a parent, guardian or other authorized individual, who becomes the responsible party for the account. The instructions set an order of priority for who may open the account: legal guardian, parent, adult sibling or grandparent of the child. For the instructions' purposes, an eligible child is under age 18 at the end of the year the election is made and has a valid Social Security number issued before the election.
The form says that for the $1,000 Pilot Program Contribution, the child must have been born in 2025-2028, be a qualifying child of the person opening the account, be a U.S. citizen and have a valid Social Security number. The instructions give the birth window as after December 31, 2024, and before January 1, 2029. If you are unsure whether a child qualifies, follow the instructions or ask a tax professional.
On June 11, 2026, the IRS posted an update for state, territorial and tribal child welfare agencies. Agencies that are a child's legal guardian follow special procedures and cannot elect the $1,000 pilot contribution.

How to fill out Form 4547 online

Part I: Parent/Guardian or Other Authorized Individual Information
First, middle and last name, SSN, home address with apartment number, city, county, state and ZIP code, foreign address fields if needed, date of birth, phone number and email address.
Part II: Child's Information (Child 1 and Child 2)
Lines 1a to 1c are the child's first, middle and last name, line 2 the SSN and line 3 the date of birth. Line 4 is the child's relationship to you; the instructions give examples such as ward, son, daughter, brother, sister or grandchild. Line 5 is the child's home address: check the box if it is the same as Part I, or complete lines 5a to 5f (and 5g to 5i for a foreign address). On line 6, check the box if you are authorized to open the Trump account for the child. For more than two children, the instructions say to attach as many copies of Form 4547 as needed.
Part III: Pilot Program Contribution Election
Line 7 has a checkbox for each child. Check it only if the child qualifies for, and you want the child to receive, the $1,000 Pilot Program Contribution.
Part IV: Consent to Disclose Information, and signature
By completing the form you authorize the IRS, Treasury and their agents to create and maintain the account and to tell other parents, guardians or authorized individuals that an account has been established. Sign and date in the "Sign Here" block. The Paid Preparer Use Only section is for a preparer if one completed the form.
How to submit Form 4547
As of October 3, 2026, the instructions describe three ways to file:
- Online through trumpaccounts.gov
- With your current-year tax return when you e-file
- By mail, to the address shown at IRS.gov/PaperReturns
The instructions say elections can be made at any time, including with a tax return.
Common mistakes
- Checking line 7 for a child born before 2025 or after 2028
- Leaving line 6 unchecked
- Applying for a child who does not yet have a Social Security number
- Forgetting the address box on line 5
- Listing a third child on the same page instead of attaching another copy
- Sending the form unsigned
