What Form 9465 is

Form 9465, Installment Agreement Request, is how individuals ask the IRS for a monthly payment plan when they can't pay the full amount shown on a tax return or an IRS notice. This template is the Rev. September 2020 edition, a 2-page PDF. As of October 3, 2026, it is still the current final version on the IRS Form 9465 page, and it is used with the Instructions for Form 9465 (Rev. July 2024).

Who uses it

The instructions say Form 9465 is for individuals who owe income tax on Form 1040 or 1040-SR, who may be responsible for a trust fund recovery penalty, who owe employment taxes from a sole proprietorship that is no longer operating, or who owe an individual shared responsibility payment. The form is not for a business that is still operating and owes employment taxes, or for people in bankruptcy or with an offer in compromise pending.

If you owe $50,000 or less, you may be able to set up the plan online instead, at a lower fee, through the Online Payment Agreement tool. If you can pay in full within 180 days, a short-term plan avoids the setup fee. Which option fits your situation is a decision for you or a tax professional.

What to have ready

  • The tax form and tax years the debt is for (for example, Form 1040 for 2025).
  • The balance due from your return or IRS notice, plus any other balances you owe.
  • The monthly amount you can afford and the day of the month you want to pay (no later than the 28th).
  • Your bank routing and account numbers if you want direct debit.
  • For Part II, details of your household, pay, vehicles, and monthly expenses.

How to fill out Form 9465 online

Part I, top section and lines 1 to 4

Enter the form number the debt relates to and the tax years or periods involved. Line 1a takes your name, your spouse's name for a joint return, both SSNs, and your current address, with foreign address boxes if needed. Check line 1b if the address is new since your last return. Line 2 is for the name and EIN of a business that is no longer operating. Lines 3 and 4 are your home and work phone numbers and the best times to call.

Lines 5 to 12, the amounts

Line 5 is the total you owe on the return or notice, and line 6 is any other balance not on line 5. Line 7 adds them. Line 8 is any payment you are sending with the request, and line 9 is what is left. Line 10 divides line 9 by 72. Line 11a is the monthly amount you propose. If it is less than line 10, line 11b lets you raise it, or you check the box and attach Form 433-F, Collection Information Statement. Form 433-F is also required if line 9 is more than $50,000. Line 12 is your monthly payment date.

Lines 13 and 14, how you will pay

Lines 13a and 13b are your routing and account numbers for direct debit. The authorization printed below them lets the U.S. Treasury withdraw each monthly payment from that account. Line 13c is a checkbox for low-income taxpayers who can't pay by direct debit. Line 14 is for payroll deduction, which needs a completed Form 2159.

Signature and Part II

Both spouses sign if the debt is from a joint return. Part II, on page 2, is completed only if all three of these apply: you defaulted on an installment agreement in the past 12 months, you owe more than $25,000 but not more than $50,000, and line 11a (or 11b) is less than line 10. It asks about your county, marital status, dependents, pay frequency and take-home pay, vehicles, health insurance premiums, court-ordered payments, and child or dependent care costs.

Where to send it

If you are filing it with your tax return, attach Form 9465 to the front of the return. If you already filed, or you are answering a notice, mail it by itself to the address in the instructions. That address depends on your state and on whether your return includes Schedule C, E, or F. The instructions say the IRS usually replies within 30 days, though requests about returns filed after March 31 can take longer. The form is mailed on paper, so check the IRS rules in IRM 10.10.1 before using an electronic signature. If in doubt, print it and sign in ink.

Fees and costs (as of October 3, 2026)

The IRS payment plans page lists these long-term plan setup fees when you apply by phone, mail, or in person, which includes Form 9465:

For low-income taxpayers, the IRS waives the fee for direct debit plans and charges $43 for other methods, which may be reimbursed if certain conditions are met. Interest and late payment penalties keep accruing until the balance is paid in full.

Common mistakes

  • Leaving line 11a blank. The form says the IRS will then set the payment by dividing line 9 by 72.
  • Choosing a payment date after the 28th.
  • Skipping Form 433-F when line 9 is over $50,000 or when you can't meet the line 10 amount.
  • Only one spouse signing a request for a joint return debt.
  • Filing Form 9465 when the online application would have cost less.
