About the Kentucky rental application
This two-page "Kentucky Rental Application" is a general tenant screening template. It gathers the applicant's personal details, everyone else who will live in the home, three previous residences, three jobs, bank and credit accounts, vehicles, references, and answers to 17 background questions, then ends with a signed authorization for credit and criminal checks. Kentucky does not issue an official statewide rental application, and this template is not a state form.
Landlords and property managers in Kentucky use it to screen applicants in a consistent way, and renters complete it when a landlord sends them the PDF.
Gather this first
- Driver's license number, Social Security number, and any former names
- Your last three addresses with rent amounts, landlord phone numbers, and move dates
- Your last three employers with supervisor names and monthly gross pay
- Bank names and balances, credit card and auto loan balances
- Vehicle make, model, color, year, and plate
- Contact information for a doctor, a lawyer, and a relative who lives elsewhere

Applicant and household
Enter First Name, Middle, Last, Birth Date, Social Security #, Driver's License #, Any Other Names You've Used In The Past, Home Phone, and Cell Phone. Under All Other Proposed Occupants, list up to four people with their birth dates and Relationship To Applicant.
Rental/Residence History
Three columns cover your Current, Previous, and Prior Residence. Each asks for street address, city, state and zip, Last Rent Amount Paid, Owner/Manager and Phone Number, and Reason for leaving, plus whether rent was paid in full, whether you gave notice, and whether you were asked to move. Add the name(s) your utilities are billed in and your From/To dates of residency.
Employment and credit
For three jobs, give Employed By, Address, Employer's Phone, Occupation, Name of Supervisor, Monthly Gross Pay, and dates of employment. Credit History asks for the institution and the balance on deposit or owed for savings, checking, a credit card, and an auto loan.
Vehicles, references, and questions
List two vehicles, including any owned by other occupants. On page 2, give name, address, and phone for a Doctor, a Lawyer, and your Nearest Relative Living Elsewhere. The printed text authorizes the landlord to contact them if you cannot be found and lets that relative remove your belongings if the apartment is abandoned. The General Information block then asks about late rent notices, smoking, bankruptcy, felonies, eviction notices, pets, lawsuits, reasons for moving, optional extra income, and how you found the listing.
Agreement and Authorization
Sign and date. By signing, you confirm your answers are true and authorize credit and criminal checks. The paragraph also says the application fee is non-refundable.
Kentucky landlord-tenant rules that apply
General information as of October 2026.
The URLTA applies only where adopted locally
Kentucky's Uniform Residential Landlord and Tenant Act (KRS 383.505 to 383.715) is not automatically statewide. KRS 383.500 lets cities, counties, and urban-county governments adopt it in full. A Legislative Research Commission fiscal note reported that 32 cities and five counties, including Louisville/Jefferson County and Lexington/Fayette County, had adopted it. A 2026 bill to rewrite the act did not advance past committee. Check with your city or county to see whether the act applies where the rental is located.
Application fees
The Kentucky URLTA does not contain a limit on rental application or screening fees.
Security deposits in URLTA areas
Where the act applies, KRS 383.580 requires the landlord to:
- Keep security deposits in a separate account used only for that purpose at a regulated bank or lending institution, and tell prospective tenants where the account is and its account number.
- Give the prospective tenant a written list of existing damage, with estimated repair costs, before taking a deposit. The tenant may inspect the unit, and both sign the list.
- Inspect at move-out and prepare a second damage list with estimated costs that the tenant can inspect and sign or dispute in writing.
A landlord who skips the separate account or either damage list is not entitled to keep any part of the deposit. If a tenant leaves owing no rent, the landlord must send notice of the refund due to the tenant's last known address, and may keep it only if the tenant does not respond within 60 days. The statute does not set a maximum deposit amount.

- Writing net pay in the Monthly Gross Pay field.
