What this New Hampshire rental application is
This two-page PDF, titled "New Hampshire Rental Application," is a general template that landlords and property managers hand to prospective tenants. It is not a form issued by the State of New Hampshire, and state law does not prescribe one official application. Page 1 collects identity, household, residence, employment, credit and vehicle details. Page 2 covers references and emergency contacts, a set of general screening questions, and a signed Agreement & Authorization.
Who uses it
Small landlords renting apartments or houses in New Hampshire use it to screen every applicant the same way. Renters use it to get a complete application ready before a showing. The form is built around one primary applicant plus a list of other proposed occupants, so each additional adult who will sign the lease usually fills out a separate copy.
What to have ready
- Street addresses, last rent paid, and owner or manager phone numbers for your current, previous and prior homes, plus move-in and move-out dates
- Employer name, address, phone, supervisor, monthly gross pay and dates for up to three jobs
- Bank or lender names and approximate balances for savings, checking, a credit card and an auto loan
- Make, model, color, year and plate for every vehicle that will be parked at the property, including other occupants' vehicles
- Names, addresses and phone numbers for a doctor, a lawyer and your nearest relative living elsewhere
- Your Social Security number and driver's license number
How to fill it out online

Page 1: applicant, occupants and history
- Applicant row: First Name, Middle, Last, Birth Date, Social Security #, Driver's License #, Any Other Names You've Used In The Past, Home Phone and Cell Phone.
- All Other Proposed Occupants: four rows, each with a name, Birth Date and Relationship To Applicant.
- Rental/Residence History: three columns (Current, Previous and Prior Residence) with Street Address, City, State & Zip, Last Rent Amount Paid, Owner/Manager and Phone Number, and Reason for leaving. Each column also asks three short questions: Is/Was rent paid in full?, Did you give notice?, and Were you asked to move? Below that are a line for the name(s) in which your utilities are now billed and From/To dates of residency.
- Employment History: Current, Previous and Prior Employment, each with Employed By, Address, Employer's Phone, Occupation, Name of Supervisor, Monthly Gross Pay and From/To dates.
- Credit History: Bank/Institution Name and Balance On Deposit or Balance Owed for a savings account, checking account, credit card and auto loan.
- Vehicles: two rows for Make, Model, Color, Year and License Plate.
Page 2: contacts, questions and signature
The References & Emergency Contacts table asks for the name, street address, city, state, ZIP and phone of a Doctor, a Lawyer and your Nearest Relative Living Elsewhere. Read the sentence printed under it: by signing, you let the landlord contact these people if they cannot locate you, and let the listed relative remove the contents of the unit if it is abandoned.
The General Information section asks about late rent notices, smoking, how long you expect to rent, bankruptcy, your move-in date, felony convictions, eviction notices, pets (type, breed, approximate weight and age), recurring problems with your current landlord, why you are moving, optional income you want considered, someone who could lend you rent money, past lawsuits, anything negative a credit or criminal check might show, how you heard about the apartment, your email address, and referrals. Put an answer in every box, even if it is "No" or "None."
Signing and submitting
The Agreement & Authorization lets the landlord run a credit and/or criminal check and contact everyone named, and it confirms that the application is not a lease. Sign in the Signature field (see how to sign a PDF online), fill in the Date, and send the PDF to the landlord or manager the way they ask, usually by email or through their rental portal.
New Hampshire fee and deposit rules
General information as of October 2026, not legal advice. Check the linked statutes for current wording and any exemptions that apply to your rental.
- Application fees: New Hampshire does not set a dollar cap on application fees. However, RSA 540-A:3 requires a landlord, before collecting any application or renewal fee, to disclose in writing the amount of the fee and whether a satisfactory criminal background and credit check is required. If the unit is not rented to the applicant, the landlord must return any amount beyond the documented cost of the background or credit check and reasonable administrative costs within 30 days. Keep that rule in mind when you read the form's line saying the processing fee is non-refundable.
- Security deposit limit: under RSA 540-A:6, a landlord may not demand or receive a security deposit larger than one month's rent or $100, whichever is greater. A landlord who holds a deposit for a year or longer must pay the tenant interest on it.
- Return deadline: RSA 540-A:7 requires the landlord to return the deposit, plus any interest due, within 30 days after the tenancy ends, with a written itemized list of any damage deductions.
Common mistakes
- Leaving the Previous or Prior Residence column blank when you did live somewhere else. Landlords often call each owner or manager listed.
- Typing annual salary into Monthly Gross Pay.
- Listing only your own car when the Vehicles section asks for other occupants' vehicles too.
- Skipping the optional income box when you have other verifiable income you want counted.
- Not keeping a copy of the signed PDF, the landlord's written fee disclosure and your payment receipt.
