About this North Dakota rental application

The "North Dakota Rental Application" is a two-page general template for renting an apartment or house in North Dakota. It is not a state-issued form; North Dakota does not publish an official rental application, so landlords may use their own. The template gathers the information most landlords check during screening: identity, everyone who will live in the unit, three residences, three jobs, accounts and debts, vehicles, references, and a set of yes or no style questions, followed by a signed authorization for credit and criminal checks.

Who fills it out

Prospective tenants complete it at the landlord's request, often before or right after a showing. Landlords and property managers in places like Fargo, Bismarck, Grand Forks and Minot use templates like this to collect the same details from every applicant. The form has one applicant section, so each adult who will be on the lease typically completes a separate copy.

Information to gather first

  • Your last three addresses with the rent you paid, the owner or manager's name and phone, and your dates at each
  • Your current and two previous employers, with supervisor names and monthly gross pay
  • Names and balances for your savings and checking accounts, a credit card and any auto loan
  • Vehicle details for you and every other proposed occupant
  • Pet details: type, breed, approximate weight and age
  • Your Social Security number, driver's license number and any other names you have used

  1. Applicant: First Name, Middle, Last, Birth Date, Social Security #, Driver's License #, Any Other Names You've Used In The Past, Home Phone, Cell Phone.
  2. All Other Proposed Occupants: up to four people, each with Birth Date and Relationship To Applicant.
  3. Rental/Residence History: Current, Previous and Prior Residence columns for Street Address, City, State & Zip, Last Rent Amount Paid, Owner/Manager and Phone Number, Reason for leaving, Is/Was rent paid in full?, Did you give notice?, and Were you asked to move? A separate line asks for the name(s) your utilities are billed under, followed by From/To dates of residency.
  4. Employment History: Current, Previous and Prior Employment with Employed By, Address, Employer's Phone, Occupation, Name of Supervisor, Monthly Gross Pay and dates of employment.
  5. Credit History: institution name and balance on deposit or owed for savings, checking, credit card and auto loan.
  6. Vehicles: Make, Model, Color, Year and License Plate, including vehicles of other occupants.

  1. References & Emergency Contacts: name, address and phone for a Doctor, a Lawyer and your Nearest Relative Living Elsewhere. Signing gives the landlord permission to contact them if you cannot be located and lets the listed relative remove your belongings if the unit is abandoned.
  2. General Information: questions on late rent notices, smoking, planned length of stay, bankruptcy, move-in date, felony convictions, eviction notices, pets, problems with your current landlord, reasons for moving, extra income you want considered (optional), a person who could lend you rent money, lawsuits, anything a credit or criminal check may show, how you found the unit, your email address, and referrals.
  3. Agreement & Authorization: you confirm your answers are true, authorize credit and/or criminal checks and contact with everyone listed, and acknowledge a non-refundable processing fee. Add your Signature and the Date.

Signing and sending it

North Dakota deposit rules that relate to this form

General information as of October 2026 from North Dakota Century Code chapter 47-16. It is not legal advice; read the statute for the full text.

  • Application fees: chapter 47-16, North Dakota's leasing statute, does not set a limit on rental application fees.
  • Deposit cap: section 47-16-07.1 limits a security deposit to one month's rent. A landlord may accept up to two months' rent from an applicant convicted of a felony, as an incentive to rent to that person, and may demand up to two months' rent from someone who had a judgment entered against them for violating a previous rental agreement. This is one reason the form's felony and eviction questions matter.
  • Pet deposit: a separate pet security deposit may not exceed the greater of $2,500 or two months' rent, and it cannot be charged for a service animal or an assistance animal a tenant with a disability needs as a reasonable accommodation.
  • Where the deposit is held and interest: the deposit must be kept in a federally insured interest-bearing account for the tenant. Interest is not required if the occupancy lasted less than nine months.
  • Return deadline: the landlord must mail or deliver the refund and an itemized list of deductions within 30 days after the lease ends and the tenant gives up possession. Withholding deposit money without reasonable justification can make the landlord liable for treble damages.
  • Move-in condition statement: section 47-16-07.2 requires the landlord to give the tenant a statement describing the condition of the premises, signed by both, when the rental agreement is made.

Mistakes to avoid

  • Answering the pet question with only a number. The form asks for type, breed, approximate weight and age, which also affects any pet deposit.
  • Leaving the felony, eviction or bankruptcy boxes empty instead of answering them.
  • Forgetting the utilities line, which landlords use to confirm where you live now.
  • Mixing up monthly and yearly pay in the Monthly Gross Pay boxes.
  • Sending the file before signing and dating page 2.
