What this Oklahoma rental application is

This template is a four-page Residential Lease Application laid out in the format of the form published by the Oklahoma Real Estate Commission (OREC) for real estate licensees and property managers. It is a general application, not a form you file with any state agency, and this copy is an older edition. OREC posts a current 2026 version of the Residential Lease Application on its contract forms page, so if your landlord or broker uses OREC forms, ask which edition they expect.

The form collects identity, rental history, employment, income and background answers for up to two applicants, plus occupants, pets and vehicles, and ends with an authorization for credit, criminal and reference checks.

Who uses it

Oklahoma renters applying for a house or apartment listed by a broker or property manager, and landlords who want a standard screening packet. Each co-applicant gets a full section of their own, so roommates or spouses can apply on one form.

What to have ready

  • A copy of photo ID for every applicant. The form says it must be submitted with the application.
  • Your Social Security number, date of birth, and driver's license number.
  • Addresses, dates, rent or mortgage payments, and landlord or mortgage company contacts for your present and previous homes.
  • Current and previous employer details, supervisor names and phone numbers, and gross monthly income.
  • Bank name, pet details (breed, sex, weight, age, spay or neuter status), renter's insurance company, and vehicle makes, models and plate numbers.

Section 1. Rental Property/Lease Information

The landlord or broker usually fills this part: Property Address, Lease Start Date, Lease End Date, Rent Amount, Security Deposit, any other deposit, and the Application and Credit Check Processing Fee per credit report. The printed terms say rent and deposits are payable in certified funds, the processing fee is not refundable, and a prepaid deposit is refunded if the application is not approved. There is also a blank for how many days you may review the lease after approval (3 days if left blank). Read these terms before you sign.

Section 2. Applicant Information

Enter your name (first, middle, last), Social Security number, date of birth, best, work and cell phone numbers, email, driver's license number, and whether a photo ID is attached. The emergency or death contact block cites Title 41 O.S. Section 130.1A and must name someone who is not a co-applicant and will not live with you. Then complete Present and Previous Address Information: whether the landlord is related to you, address, how long, rent or mortgage payment, landlord or mortgage company, phone, and reason for leaving.

Employment history and background questions

List your current and previous employer, position, address, how long, gross monthly income, supervisor and phone, plus other income and its source. Answer the yes or no questions on bank accounts, bankruptcy (with discharge date), evictions, broken leases, felony or misdemeanor convictions, and lawsuits for unpaid rent or property damage, adding an explanation for any "yes."

Section 3. Co-Applicant Information

A second applicant repeats the same identity, address, employment and background fields.

Section 4. Other Information

List other occupants with ages and relationships, up to three pets, whether any smokers will live there, whether you carry tenant's insurance and with which company, and up to four vehicles, trailers, boats or motor homes to be stored at the property.

Section 5. Acknowledgement, Agreement and Authorization

By signing, applicants confirm the information is true, authorize verification and credit reports, and agree the application is kept whether or not approved. The broker signs as well, and the office-use lines record when deposits and the processing fee were received. You can sign the PDF online before downloading.

Oklahoma rules worth knowing

General information as of October 2026, based on the Oklahoma Residential Landlord and Tenant Act as compiled by OREC:

  • Application fees: the Act does not contain a provision capping rental application or screening fees. The amount is set by the landlord and written into Section 1.
  • Security deposits: under 41 O.S. Section 115, deposits must be held in an escrow account with a federally insured institution in Oklahoma. The Act sets no maximum amount. The landlord must return the balance, with an itemized written statement of deductions, within 45 days after the tenancy ends, possession is delivered, and the tenant makes a written demand. If the tenant makes no written demand within six months after the tenancy ends, the deposit reverts to the landlord.
  • Screening: under 41 O.S. Section 113.3, a landlord may not deny a tenancy because an applicant or household member is a victim of domestic violence, sexual violence or stalking, or because the applicant ended a previous lease for that reason.

Common mistakes

  • Listing a co-applicant or future roommate as the emergency contact, which the form does not allow.
  • Forgetting to attach photo ID for every applicant.
  • Leaving the lease review days blank without realizing the form defaults to 3 days.
  • Paying the processing fee in cash or personal check when the landlord requires certified funds.
  • Skipping explanations next to "yes" answers in the background questions.

Frequently Asked Questions

No state agency requires one rental application. This template follows the layout of the Oklahoma Real Estate Commission's Residential Lease Application, which OREC publishes for licensees, and OREC posts the current edition on its contract forms page.

The Oklahoma Residential Landlord and Tenant Act does not include a cap on application or screening fees. On this form, the landlord writes the processing fee per credit report in Section 1, and the form states it is not refundable.

The Act does not set a maximum deposit. It requires the deposit to be held in an escrow account in a federally insured institution in Oklahoma, under 41 O.S. Section 115.

Within 45 days after the tenancy ends, possession is delivered, and the tenant makes a written demand, with an itemized statement of any deductions. The tenant must make that written demand within six months, or the deposit reverts to the landlord.

The form asks for a copy of photo identification for every applicant and the processing fee. Landlords often ask for proof of income as well.
